MCOOL Information and Affidavit Forms

Various industry groups have developed a standard affidavit form to be used to verify the country of origin for cattle. These forms have received approval by the United States Department of Agriculture and when properly used, will provide verification of the country source of cattle. The forms can be used to verify origin of U.S. cattle or can be also be used to verify origin of imported cattle.
